MEDICAL

 

The Austin/Travis County Health Department has Community Health Centers in the Austin area and Travis County. The Centers handle WIC, clinic cards, dental referrals, prenatal care, immunizations and family planning.

 

A Medical Assistance Program (MAP) Clinic Card is required. The MAP is a service of the City of Austin Health and Human Service Department.  MAP provides access to health care for City of Austin and Travis County residents who meet eligibility criteria. 

 

MAP eligibility guidelines:  (1) Travis County residents with family income at or below 100% of the Federal Poverty Guidelines, and who meet asset guidelines; Includes Williamson County residents living in Austin.  City of Austin residents on Medicaid will not qualify for MAP.  (2) Older adults and people with disabilities whose income is at or below 200% of the Federal Poverty Guidelines, and who meet guidelines.  City of Austin residents on Medicaid will not qualify for MAP.  (3) Residents who receive Medicare and meet the income and asset guidelines may qualify for limited MAP benefits.  

  

For Austin residents, call 972-5300 to be screened for Map eligibility.

For non-Austin residents call your local rural community centers (pg. 84).

 

The Community Health Centers offers sliding scale fees for person without MAP card. 

 

What do I need to bring to a MAP eligibility interview?

 

1)      Birth Certificate, Certificate of Naturalization, Permanent Resident Card, Work Authorization Card or Passport for all Family members

2)      Texas Drivers’ License or picture ID for adults

3)      Social Security Cards (if applicable) for all family members

4)      Current utility bill (last 30-45 days)

5)      Proof of Income for last 4 weeks (example: check stubs, employment form, Social Security Award Letter for 2003, financial support statement)

6)      Medicare, Medicaid or other Insurance card of letter (if applicable)

7)      Latest Bank Statement (if applicable)

8)      Tax receipt or Property appraisal (property other than homestead)

9)      Proof of sponsor’s income (if individual has permanent residency for less than 5 years)
